Benefits
- How can I get an employment verification for purposes of obtaining a loan or mortgage or to apply for social services?

The company has contracted with a national employment verification service, The Work Number for Everyone, a service of TALX Corporation. All requests for employment verifications and salary information must be submitted to the The Work Number for Everyone. This service provides employment and salary information to verifiers either through a touch-tone phone or the internet. There is no cost to our employees for this service.
Verifiers can contact TALX by either calling 1-800-367-5690 or via the internet at www.theworknumber.com. Social Service Agencies or Child Support Offices may call 1-800-660-3399. Our company code is 10426. You may obtain further information regarding this service by clicking here or you may call a TALX Customer Service Representative at 1-800-996-7566.
- I may need to be out of work for an extended period due to an illness. Where can I obtain more information about my rights under the Family Medical Leave Act (FMLA)?

Your employee handbook or Regional Policy Manual contains information regarding Family Medical Leave (FML), such as eligibility, reasons for taking FML, and provides the steps that must be taken to apply for FML. We do not automatically designate leave as FML. Employees must complete and submit the FML application and medical certification forms within the required timeframe (14 days) to be approved for FML. In most states, FML provides up to twelve weeks of unpaid, job-protected leave and ensures continuation of existing health coverage to eligible employees who meet the requirements for taking the leave. In states where the law extends the leave beyond 12 weeks, the state law would apply (example: Connecticut allows up to 16 weeks of unpaid leave). - How can I find out what benefits are offered to me as an employee of the company, such as health insurance, life insurance and 401(k)?

- Can I continue my benefits once I leave the company?

Any health, dental and vision benefits in which you were enrolled as an employee may be continued through COBRA at 102% of the premium price. Once your employment with the company ends, you will be sent a COBRA letter and application, along with the applicable rates, so that you may enroll. Should you return to work, contact your General Foreperson or Supervisor immediately to determine eligibility for re-enrollment as an active employee. - Who can I contact about my garnishment or child support payroll deductions?

Any deductions taken from your paycheck are done so only under an Order from the Court. If you feel a garnishment or child support deduction is in error, please contact the issuing Court to resolve any issues. Please note that deductions cannot be stopped without a direct Order from the Court to release the previous Order. Any other questions should be directed to the Wage Attachment Department at the Home Office at 1-800-248-8733.
Payroll
- How do I sign up for Direct Deposit?

Please contact your immediate Supervisor to obtain a Direct Deposit form or click here for a printable version. Complete the form and send a copy of it and a voided check to the Home Office, attention of the Direct Deposit Office.
- How do I change the address on my paycheck?

Address changes should be sent to the Payroll Department at the Home Office. Please include your name, last four digits of your social security number and your new address. You may also e-mail your request to JoAnne Howell at johowe@asplundh.com.
- I have not received my W-2 form. How can I get another copy?

Please call the Payroll Department in the Home Office at 1-800-248-8733, ext. 4314 to obtain a copy of your current W-2. If you need a W-2 from a prior year, please contact the Tax Department in the Home Office at 1-800-248-8733.

- Can I order “Asplundh” clothing?

Clothing with our Companies’ logos is available to current employees only. Non-employees can order many of the same clothing items without our Companies’ logos by contacting the Lion Uniform Group at 1-800-543-9698.
Employees can now order “Asplundh” clothing online. The new e-Store is available for those of you paying by credit card ONLY. Follow this link, www.lionuniformgroup.com/asplundh/, or go to the “Quick Links” section of the Procurement Services site on the Employee Portal. For further information, contact your General Foreperson, Supervisor, Regional Office or the Procurement Services Department (Purchasing) at the Home Office.
- How do I get the current Tool Supply ordering forms?

Many of our order forms are available on the Procurement Services site on the Employee Portal. In order to access this information, you must have an active “@asplundh.com” e-mail account and be able to connect to the network through VPN software installed on the computer. Orders may also be placed by submitting the XX-14A ordering form. For further information, contact your General Foreperson, Supervisor, Regional Office or the Procurement Services Department (Purchasing) at the Home Office.
- Can I order items not listed on the pre-printed order forms?

YES! Submit your requests on the “PUR2 – Special Order” form. Be sure to complete all areas of the order form and include as many details as possible. As always, all orders are subject to your “Regional Ordering Approval Process”. If you are not authorized to place orders, submit your request(s) to the appropriate person(s) within your Region for approval and they will forward the order to Procurement Services.
- How can I get one of those cool Asplundh toy bucket trucks for myself?
